Required Forms for Enrolling Students in 2025 Youth Leadership Summit

Grantees who are allocated slots for the Youth Leadership Summit are responsible for completing an enrollment form with student information, chaperone information, and director approval.

These are the required forms to upload as part of enrollment (click links to download):

  1. Parent-Guardian Consent Form / Formulario de Consentimiento del Padre / Guardián Legal (1 per Student)

  2. Media Release Form / Formulario de Autorización para Sesiones de Grabación de Video y Toma de Fotos (1 per Student)

  3. Student Pledge / Student Pledge (Español) (1 per student)

  4. GEAR UP Director Student Approval Form (1 per grant)

  5. GEAR UP Director Chaperone Approval Form (1 per grant)
